Job Description
The Outreach and Intake Specialist supports patient access to care by building referral relationships, responding to inquiries, completing intake screening, and coordinating the first steps of treatment. The role combines community outreach, front end intake coordination, and administrative documentation. Work is performed in an office setting and in the community, with local travel required.
• Respond to phone and email inquiries, provide program information, and schedule initial appointments• Complete intake screening, collect required demographic and clinical information, and route patients to appropriate services
• Verify eligibility and coverage when applicable, coordinate prior authorizations when required, and support accurate intake documentation
• Support patient engagement, appointment reminders, follow up after missed appointments, and warm handoffs to clinical staff
• Maintain accurate, timely documentation in the electronic record system and protect patient privacy in accordance with HIPAA
• Assist with front desk duties as needed, including scanning records, releasing information with proper authorization, and maintaining organized files
Credentials Needed
• High school diploma or GED
• At least 1 year of experience in healthcare, behavioral health, customer service, intake, scheduling, call center work, or a related administrative role
• Strong communication skills, including professional phone presence and clear written documentation
• Basic computer proficiency, ability to learn electronic health record workflows, and comfort using email and calendar tools
• Knowledge of confidentiality standards and willingness to follow HIPAA requirements and company policies
• Ability to pass required background checks for behavioral health settings, including fingerprint clearance processes if required for the role
• Valid driver license and reliable transportation, ability to drive locally for outreach activities
• Ability to lift up to 25 pounds, sit and stand for extended periods, and work in both office and community environments
• Current CPR and First Aid certification.
• Level 1 Fingerprint Clearance Card
Preferred qualifications:
• Experience with Arizona Medicaid programs such as AHCCCS, or experience verifying eligibility and benefits
• Experience in behavioral health, substance use disorder treatment, crisis services, or recovery oriented systems of care
• Bilingual English and Spanish
• Familiarity with referral coordination, community resource navigation, and trauma informed engagement
